FUNCTIONALITY VS ALGOMETRY AS A MEASURE OF THERAPEUTIC SUCCESS IN NON-SPECIFIC CHRONIC LOW BACK PAIN

Abstract

Goal: systematic review of scientific literature in order to compare the variety of evaluation tools that can be used to calibrate the success of the multiple existing therapeutic possibilities for one of the most prevalent alteration among our patients: non-specific chronic low back pain. Method: systematic research over main biomedical data-bases: MEDLINE, EMBASE, PASCAL, PSYCHOINFO, BIOSIS, CINAHL, COCHRANE DATABASE e IME using the following key words: low back pain, chronic, disability scales, outcome measures, algometer, health status measures, treatment efficacy, pressure/mechanical pain threshold. We selected those articles that recommend a certain treatment being based in metaanálisis, systematic reviews, and randomized controlled trials, and that also pay special attention to the most relevant European clinical practice guideline about low back pain. Results: most of the selected studies recommend the use of different parameters such as pain severity and a variety of indexes and scales including functionality parameters: disability, autoevaluation pain scale, and even other: strength, mobility, medication. In contrast, pressure algometry is not used as an evaluating measure in most studies; nevertheless, there are some studies that justify its utilization when low back pain is associated with muscular mechanical hiperalgesia,though notable controversies exist about its correlation with the results of pain scales and disability questionnaires.
conclusions: to stress the importance of measurement to properly show the effects of treatment programs in every patient life area approaching to the new health and disease holistic model has to be stressed. In this context, the use of pressure algometry leads to notable controversies, thus seeming its use fundamentally advisable for re-search and mainly for research groups.
