Abstract

This work was carried out on 172 patients with tension-type headache and 55 normal control subjects to determine the relationship between cervical spine muscle guarding, low-set shoulders, and cervical spine instability. A huge majority of the patients with tension-type headache were also found to have straightened cervical spine. The flexor muscles of the head and neck prevent the cervical spine from the lordosis, and their sustained chronic contraction may be a principal cause of a straightened contraction. The low-set shoulders were frequently seen in patients with tension-type headache, and they may result from traction of the brachial plexus, which would enable the pain to  rise in the neck and shoulders. Cervical spine instability was quite unlikely to be present in patients with tension-type headache. Our results suggest that both the cervical spine muscle guarding and low-set shoulders may play an important role in the pathogenesis of tension-type headache and its accessory symptoms.
