Introduction: chronic fatigue syndrome is a disabling disease with a high incidence in young adults whose main characteristic is persistent fatigue. Despite the fact that there is no effective curative treatment, the prognosis is unfavorable if it is not treated. Objective: to recognize and analyze the available evidence about the effectiveness of physical exercise in patients with chronic fatigue syndrome. Material and method: a literature search was carried out between February to May 2019 in the following databases: Scopus, Medline and PEDro. The keywords used were "Fatigue Syndrome, Chronic" and "Exercise".The selected studies were randomized clinical trials writ-ten in English or Spanish. To include them, the patients had to be adults of both sexes diagnosed with chronic fatigue syndrome whose intervention was physical exercise. Results: ten randomized clinical trials were selected for in-depth analysis. Qigong therapy and non-protocolized gradual exercise, as well as protocolized gradual exercise, revealed an important improvement in fatigue values, the latest also showing improvement in physical function; lastly, isometric exercise had different results between studies, or they were not significant. Conclusions: this systematic review found a significant improvement for fatigue values in patients diagnosed with this syndrome when per-forming Qigong and gradual exercise therapies (protocolized and non-protocolized).
